I understand that shipping Cremated Remains and/or Memorial items without signature required can result in loss, theft or damage of this package. I waive the requirement for a signature upon delivery of my Pet’s Cremated remains and/or Memorial items.

I accept full responsibility for the package(s) once they are delivered to the address provided. This includes any loss, theft, or damage that may occur after delivery.

I acknowledge that the delivery service provider may consider the package(s) delivered and complete once left at the specified address, even in the absence of my signature.
